- Patients have the option to fill out the medical questionnaire in advance and bring it with them.
- For the handwritten version, please download and print the questionnaire, fill it out, and bring it with you on the day of your consultation.
- For those who prefer to type their responses, a downloadable Word format is available. You can edit and enter your information on the computer, then print the completed form and bring it with you.
