Hospital Admission


On the day of the hospital admission, please come to the hospital by 10:00 am and follow the instruction at the Reception Counter on the first floor.

Required Items for Admission

  • Patient Card
  • Health Insurance Card
  • Hospitalization deposit
  • Consent forms
  • Letters of Commitment
  • * Hospitalization deposit (50,000 yen) will be applied to the final billing on the day of discharge.
  • * Application form (is also a letter of commitment) and a Consent Form for the special room usage (that incurs an extra charge) are handed to you at the outpatient consultation counter on the day of admission

    Please submit these two forms signed and sealed to the Reception on the first floor. At least one guarantor is required. A Consent Form for the special room usage is required only if you request a private room.

Our staff will escort you when required paperwork is accepted.

  4. Shampoo and conditioner
  5. Bath towels

  7. A box of tissues
  8. Tea cup/mug
  9. Slippers or shoes for indoor use

  • * Personal care amenities are also available (from 1 to 7 of the above listed items) on a rental basis from our vendor. Further information is available on an information sheet.
  • * Hospital is not liable for any stolen personal items. Please refrain from bringing in expensive jewelry or high-end products or a large sum of money to the hospital. If you are scheduled for a surgery, different items are required. Please consult the attending nurse.


Diapers are provided from us. Cost of used diapers will be billed. In principle, other diapers are not accepted.

Hospital Rooms

Some of our hospital rooms are more specious for extra comfort. These private rooms are available for an extra charge. (Some exception may apply)

Hospital Room Facilities

TV and Refrigerator
A TV and a refrigerator are located on each bedside. These charges are payable by pre-paid card issued from the hospital. Private room charge includes free use of TV and refrigerator.
A pre-paid card vending machine is located at the nurses’ station on the second floor.
Please be respectful to other patients and always use earphones.
Talking over a mobile device is permitted only in designated areas.
Safety Box
A safety box is located in the drawer of a bedside cabinet.


Our hospital meals are prepared with seasonal ingredients reflecting spring, summer, autumn or winter.

Meal Time

Breakfast at 7:30 am, Lunch at noon, and Dinner at 6:00 pm
Meals are prepared based on your medical condition. For meals and beverages that are not provided from the hospital, please consult attending physician or nurse in advance.


Billing System

  • Hospital billing at discharge
    A hospital bill is issued on the day of your discharge. Please make the payment at the casher’s counter located on the first floor before discharge.
  • We accept credit cards(Since May 2014)
    (VISA, MasterCard, SAISON, UC, JCB, AMEX and Diners)

Cashier’s Counter Hours

Weekdays 9:00 to 17:00 Saturday 9:00 to 12:00

Billing Amount

  • If you have Japanese health insurance such as the national health insurance or social insurance, your medical charge shall be calculated based on a self-pay ratio defined by the insurance policy.
  • If you have NO Japanese health insurance, the full expenses shall be charged. The amount is roughly estimated to 130 percent of charge for a healthcare service provided by health insurance.

Sundays and Holidays
9:00 to 20:00

All visitors are asked to observe visiting hours for the safety and security of our patients.
Before going to the room, please come to the reception at the nurses’ station of the second floor.
Visitors are escorted to the room after filling out a visitor information form.

Please be informed that
  • Flexible visiting hour may apply depending on the medical condition, surgery, and examination schedules. Please consult the attending nurse.
  • All visitors are asked to obtain a permission at the nurses’ station on the second floor before entering the room.
  • As young children are vulnerable to infections, visitors are strongly advised not to bring them unless it’s really necessary.
  • Pets are not allowed.
  • Anybody with a cold is strongly advised not to visit patients.
  • On the day of the surgery, please make it a brief visit.
  • Please be aware that there is NO PARKING SPACE at the hospital.
  • Smoking is prohibited on all areas of the hospital.
